At risk of losing your rental housing or becoming homeless?

Eligibility Requirements

• No other housing options
• Lacks financial resources
• No support network to obtain immediate housing or remain in existing housing
• Household income at or below 50% of Median Income (AMI) which is $27,900 for a family of four

Participants being referred should meet one or more of the following risk factors:

• Imminent eviction within 2 weeks from a private dwelling
• Discharged within 2 weeks from an institution in which the person had been a resident for more than 180 days including prison, mental health institution, hospital
• Housing condemned by housing officials
• Sudden and significant loss of income
• Mental health or substance abuse issues
• Physical disabilities and other chronic health issues, including HIV/AIDS
• Head of household completing education or job training program
• Current or past involvement with child welfare, including foster care
• Pending foreclosure of rental housing
• Recent traumatic life event, such as death of a spouse or recent health crisis
